The National Home Care Medicine Practice Directory increases access to home-based primary care that offers high-quality, convenient, at-home services for patients. Patients, families, doctors, hospitals, and insurance plans can use the powerful locator tool to search the online directory of practices that specialize in home-base medical care by ZIP code or Keyword. The Directory includes other important information, such as hospital affiliations, in-home services and urgent visits provided. The ground-breaking Home Care Medicine Provider Directory is one component of work supported by a four-part, coordinated grant from The John A. Hartford Foundation's Moving and Scaling Home-Based Primary Care Phase III, awarded to four collaborative organizations: Home Centered Care Institute, The National Home-Based Primary Care Learning Network, Duke-Margolis Center for Health Policy and AAHCM.
