Many payers and providers are outspoken about being their members’ or patients’ “health partners.” By providing HBPC services, they can fulfill this promise to vulnerable, high-risk and homebound adults.

Forward-looking healthcare leaders are meeting the needs of the invisible homebound head-on with innovative, patient-centric, and holistic solutions that combine integrated care models and technologies with an ages-old medical practice—house calls by physicians or other professionals. These home-based primary care (HBPC) teams provide desperately needed primary/palliative care and social services to more than 100,000 high-risk homebound patients.

HBPC is the best, patient-centric solution to homebound, chronically ill patients.

The “Triple Aim” conceived by the Institute for Healthcare Improvement as imperative for improving the U.S. healthcare system established three universally accepted goals to be pursued simultaneously:

  • Improving the patient experience of care (including quality and satisfaction)
  • Improving the health of populations
  • Reducing the per capita cost of health care

HBPC is vital for driving progress toward the first and third goals for the fastest growing segment of the population—homebound adults. HBPC programs epitomize patient-centric care, which the Institute of Medicine defines as “providing care that is respectful of, and responsive to, individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions.”

Home-based care provides unique opportunities to identify and respond to the needs of individual beneficiaries and families. When HBPC providers form relationships built on compassion and trust, patients are more likely to follow care protocols and to receive appropriate palliative, behavioral, or hospice care.
